Overview
DPP is a group program that helps people with pre-diabetes delay or even prevent the onset of type 2 diabetes by assisting individuals in eating healthier, becoming more physically active, and losing weight.
What is pre-diabetes and diabetes? Diabetes is a chronic disease that causes blood sugar levels to rise higher than normal. Diabetes affects more than 29 million people. Approximately 84 million Americans (more than 1 in 3) have pre-diabetes, of which 90% do not know they have it. Prediabetes occurs when blood sugar levels are higher than normal but not high enough to receive a diagnosis for type 2 diabetes. Diabetes has no cure, but prediabetes can be prevented through lifestyle changes. Without intervention, 25% of people with pre-diabetes will develop type 2 diabetes within three to five years.
HCD International Diabetes Prevention Program
In the HCD International Diabetes Prevention Program, a trained lifestyle coach facilitates a small group of adults to discuss behavior changes that can improve the health of participants. The program consists of 26 one-hour sessions delivered over the course of one year using a combination of distance learning (using a telehealth platform) and in-person class sessions.
Throughout our program, you will learn essential skills and develop lifestyle habits to help you lose weight and prevent diabetes. We deliver with cultural sensitivity so the guidance is culturally relevant.

In our program you will:
- Work with a trained lifestyle coach to develop realistic, long-term lifestyle habits.
- Learn how to eat better, reduce portion sizes and fat in the diet, and prepare meals in a healthier way.
- Learn ways to increase physical activity and lose 5-7% of body weight in order to decrease blood sugar, cholesterol, and blood pressure.
- Learn stress management techniques.
