Jacqueline Harley serves as the Outreach Lead for HCD International’s Diabetes Prevention Program and Diabetes Self-Management Training. Previously, Ms. Harley worked as the Lead Case Manager for the Hospital-2-Home project at the University of Maryland’s Prince George’s Hospital Center. Ms. Harley also served as the Assistant Director of Medical Mall Services of Mississippi, LLC, one of the Centers for Medicare & Medicaid Services’ (CMS) Accountable Care Organizations (ACO). She served in Special Assistant/Project Management positions working to improve care transitions by reducing hospital readmissions, developing and initiating interventions to provide better care for patients, better health for communities, and lowering the cost of healthcare. Ms. Harley worked at Health Care Dynamics, Incorporated, as a CMS contractor, and served as the Technical Advisor, Project Manager and Senior Communication Manager on the “Every Diabetic Counts” project. She retired from CMS with over 24 years of experience in quality improvement, health care disparities and health promotion and disease prevention. During her tenure at CMS, Ms. Harley served as the Technical Advisor, Special Assistant, Government Task Leader (GTL) and Team Leader, to the Quality Improvement Organization program in the Office of Clinical Standards and Quality. Ms. Harley has a wealth of knowledge and experience working with vulnerable populations in the community and hospital setting.
